Should my application be accepted, I agree to be bound by the policies and procedures of Destiny Kids Ministries, and refrain from unscriptural conduct in the performance of my services on behalf of the church. I also understand that by agreeing to this statement I am committing to serve no less than four months as a Destiny Church volunteer within the Destiny Kids Department.
